Be sure to bring with you to your appointment:

• Your health insurance card. We cannot see you without it.
• A COMPLETE LIST of your medications including doses. This is VERY IMPORTANT.
• Cash, check, VISA or MasterCard payment for your co-pay or deductible.
• Your referring or primary care doctor's name, address and phone number.
• Your pharmacy's phone number and address.
• If you have not had a cholesterol check in over a year, please come in a FASTING state (no food for 12 hours) and we can check your cholesterol levels at your visit. (Or, you may choose to come back another time to do the blood test.)
• Consider bringing a list of a few questions to ask the doctor.

We request that you notify us at least 24 hours in advance if you are unable to make your appointment. We reserve the right to charge a cancellation fee for missed appointments. We respect your time -- we'd like to ask the same of you.

Directions:

• From 5 North, take Encinitas Boulevard exit. Make a left off the freeway onto Encinitas Boulevard. Proceed past El Camino (approximately 2 miles). Make a RIGHT on Manchester Avenue and an immediate RIGHT into the parking lot. Our building is the first LEFT (4401 building). Park in the lot on the left. The entrance is within 10 feet.
• From 5 South, take Manchester Avenue exit and make a left. Proceed about 1/2 mile, then make another right on Manchester Avenue. Proceed about 2 miles (residential area). Our office will be on the left before Encinitas Boulevard. Pull into the parking lot and make an immediate left to the 4401 building. You will see our office right there.
